Haven't heard back from my fender bender in Indiana! Who do I contact?

I was in a fender bender almost two months ago in Highland, IN in Lake County. I was rear-ended, there is damage to the car but I wasn't hurt. Police came, we filled out what he called "proof of insurance" forms then were supposed to be notified and I haven't heard anything yet. What do I do now? I reported it to my insurance, although he was at fault and he did have insurance.
